Galadent will be attending our club again for our mouthguard requirements. If your child/children are requiring a new mouthguard, we would ask you to support our club by taking up this opportunity, as the club receives a $10.00 rebate per mouthguard.
If you are wanting a mouthguard for your child/children, PLEASE FOLLOW THE STEPS AS BELOW:-
Go to www.galadent.com.au to book in your child/children’s appointment. You are required to book an individual appointment for each child. Once you have completed your booking/s a confirmation email will be sent to you.
You are also required to complete Galadent’s online ‘Mouthguard Form’. Please click on ‘Mouthguard Form’, then ‘Download’ and SAVE the form before you complete any details. Some browsers will not save your typed information unless you have done this first. Please print BOTH pages (no need to staple them together) and bring it with you to your child/children’s appointment.
Parents, please ensure that you have made your appointment/s and completed a mouthguard form. Without completing both of these steps, it makes it difficult to run efficiently, so your assistance would be greatly appreciated.
All the types, colours and costs of the various mouthguards are outlined in the online ‘Mouthguard Form’. If required, Galadent can assist you on the night in selecting the correct type.
Payment is required on the night, by either completing your credit card details on the form, paying in cash or a cheque (made payable to Galadent Dental). If paying with cash or a cheque, please place this in a sealed envelope with your child’s details on the front.
The mouthguards will be delivered back to the club within 2-3 weeks, in time for Winter sports. The club will notify you on when and where to collect your child’s mouthguard.
An itemised account/receipt will be emailed to you at the time of delivery, so please ensure that you have completed your email address. A legible email (if handwritten) would be appreciated.
The receipt will enable you to make a claim through your private ancillary Dental cover. As Galadent are a HBF Members Plus provider, parents insured with HBF will be entitled to a higher rebate.
